IMPORTANT NOTICE REGARDING ANNUAL FORMS 1095-C
Consistent with previous years, the 2025 Form 1095-Cs have been prepared to provide required information regarding the offers of coverage that have been made to full-time and other eligible employees. However, in accordance with IRS Notice 2025-15, the annual Forms 1095-C are no longer required to be mailed, but instead are available and will be provided within 31 days from any written request. Such request shall be made via email or regular mail to the following address:
